Key Responsibilities:

  • Contribute to the creation of individualized care plans for residents and assess medical and nursing interventions to ensure alignment with the care plan and resident preferences.
  • Oversee the planning, scheduling, and updating of the Minimum Data Set (MDS), including the execution of Care Area Assessments (CAA) and Triggers.
  • Conduct daily rounds to verify that nursing staff are fulfilling their responsibilities.
  • Review nursing documentation to ensure clarity and thoroughness regarding the care provided and the resident's responses.
  • Foster positive working relationships with all facility staff to meet resident needs effectively.
  • Engage in discharge planning reviews and prepare reports for the Care Plan Committee as required.
  • Delegate tasks, provide training, evaluate performance, and support RN, LPN, and CNA staff.
  • Deliver direct nursing care as necessary.
  • Assist the Director of Nursing Services and step in as needed.
  • Participate in facility evaluations conducted by authorized governmental bodies.
  • Recommend improvements or changes in procedures, policies, training, reference materials, and nursing practices to ensure the highest quality of patient care.
  • Attend continuing education sessions to stay updated on industry changes and lead in-service training for nursing staff.

Qualifications:

  • Must hold a nursing degree from an accredited institution.
  • Must possess a current, unrestricted RN license in the state.
  • Minimum of one year of supervisory experience in a healthcare environment is required.
  • Training in rehabilitative and restorative nursing practices is essential.
  • Familiarity with nursing and medical protocols, as well as regulations governing nursing care facilities.
  • Experience with Electronic Medical Records and documentation systems is preferred.
  • Maintain confidentiality of all resident care information in compliance with HIPAA regulations.
