About the Role

We are seeking a highly skilled and compassionate Case Manager to join our Outreach Services team at People Assisting The Homeless (PATH). As an Outreach Case Manager - Encampment Resolution, you will play a critical role in assisting individuals experiencing homelessness to break the cycle of homelessness and achieve stability in their lives.

Key Responsibilities
  • Conduct thorough intake and needs assessments for clients, developing individualized service plans that address housing goals, income maintenance, and other personal objectives.
  • Provide referrals, linkages, and support to resources that help clients achieve their service plan goals, including healthcare, social services, and vocational support.
  • Complete program-specific assessments, such as the VI-SPDAT, to evaluate client strengths and challenges.
  • Maintain accurate and up-to-date client files, adhering to program and PATH policies.
Requirements
  • Two years of experience in healthcare, case management, homeless services, or a related field.
  • Excellent analytical, communication, and problem-solving skills.
  • Ability to work effectively in a team environment and maintain professional boundaries.
  • Valid driver's license, reliable transportation, and proof of insurance.
  • Ability to qualify for PATH's insurance coverage.
