Join Our Team as a Home Health LPN

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Pinnacle Home Care. As a Home Health LPN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role

As a Home Health LPN, you will travel to patients' homes to provide skilled nursing care and perform a variety of tasks as outlined in the patient's plan of care. Your responsibilities will include:

  • Providing skilled nursing care under the direct supervision of a Registered Nurse (RN)
  • Performing selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care
  • Responsible and accountable for making decisions based on your nursing experience and educational preparation
  • Timely completion of patient documentation
Qualifications

To be considered for this role, you must have:

  • A current State LPN or LVN License
  • IV Certification
  • Graduate of an accredited practical nurse or vocational nursing program
  • At least one year of nursing experience, preferably in community/home health or medical/surgical settings
  • Valid CPR certification
  • Good verbal and written communication, and organization skills
  • A licensed driver with an automobile that is in good working order and insured per the organization's requirements
