LPN III - Home Health Field Job Description

Job Summary:

We are seeking a skilled Home Health Field LPN to join our team at BayCare HomeCare. As a Home Health Field LPN, you will provide crucial care to patients in their homes, promoting their well-being and independence.

Responsibilities:

  • Deliver patient care to assigned patients, providing care for clients as ordered, including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse.
  • Identify, implement, and maintain appropriate teaching plans for patients and caregivers under the direction of a Registered Nurse.
  • Follow the plan of care and collaborate with the Registered Nurse regarding patient response to treatment and change in status.
  • Utilize nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.

Requirements:

  • BLS (Basic Life Support)
  • LPN (Registered Nurse) License
  • Driver's License State of Florida (Florida driver's license is required within 30 days of hire)
  • Required Associate's or Diploma Nursing
  • Required 1 year Licensed Practical Nurse
  • Preferred 1 year Home Care
  • Preferred IV Certification
