Quality Assurance Coordinator

Providence Care Center is seeking a skilled MDS Nurse to join our leadership team. As a key member of our team, you will be responsible for ensuring the accuracy of our resident assessments and care plan optimization.
Key Responsibilities:
- Conducting and coordinating the development and completion of resident assessments (MDS) by current rules, regulations, and guidelines
- Collaborating with the interdisciplinary team to develop individualized resident care plans
- Monitoring and documenting resident progress and responses to care plans
- Participating in the Quality Assurance and Performance Improvement (QAPI) process
- Communicating with residents, families, and other healthcare providers to coordinate optimal care delivery

Requirements:
- RAC-CT (Resident Assessment Coordinator-Certified) certification is required
