LPN Ambulatory Care Job Description

Overview

We are seeking a highly skilled and compassionate licensed practical nurse (LPN) to join our team at Amedisys, a leading provider of home health, hospice and high-acuity care. As an LPN Ambulatory Care Specialist, you will have the opportunity to make a meaningful impact on the lives of patients and their families by providing much-needed care in their homes.

Responsibilities

  • Verify plan of care with the RN prior to each visit.
  • Teach patients and caregivers self-care techniques.
  • Review all medications and perform a medication reconciliation at each visit.
  • Prepare equipment and materials for treatments using the aseptic technique.
