Vocational Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health LPN, you will provide professional nursing services to patients in their homes, focusing on rehabilitative needs, preventative care, and holistic approaches to health.
Key Responsibilities- Provide skilled nursing interventions in the treatment of patients' illnesses, rehabilitative needs, and preventative care.
- Utilize a systematic, individualized goal/outcome-driven approach in implementing the nursing plan of care.
- Maintain contact with patients, physicians, clinical managers, and other healthcare team members regarding patient/family needs and status changes.
- Prepare, submit, and maintain documentation as required by the Company and/or facility.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
