About the Role:
CenterWell Home Health is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Key Responsibilities:
- Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations, applicable licensure/certification requirements, and CenterWell Home Health policies and procedures.
- Deliver skilled nursing interventions to address patients' medical, rehabilitative, and preventative care needs.
- Utilize a holistic approach to provide problem-specific therapeutic interventions, teaching, and training activities in accordance with the patient's plan of care.
- Maintain effective communication with patients, physicians, clinical managers, and other healthcare team members regarding patient needs and status changes.
- Participate in care coordination activities and discharge planning as necessary.
- Ensure confidentiality and maintain the highest standards of professional conduct.
Requirements:
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
