Job Summary

We are seeking a highly skilled Clinical Pharmacy Specialist to join our team at MD Anderson Cancer Center. As a key member of our pharmacy department, you will play a critical role in providing exceptional patient care and contributing to the development of our clinical pharmacy services.

Key Responsibilities
  • Clinical Practice
    • Develop and maintain an active clinical pharmacy practice on assigned services, participating in inpatient rounds and outpatient clinic coverage, including on-call and weekend coverage as assigned.
    • Collaborate with the multidisciplinary healthcare team to optimize pharmacotherapeutic aspects of patient care.
    • Manage medication therapy for individual patients, developing, recommending, implementing, monitoring, and redesigning drug therapy regimens to optimize therapy.
    • Manage daily patient medication problems and emergency situations, documenting activities in the electronic health record.
    • Provide timely follow-up on the clinical effects of medication regimens, making adjustments as necessary to achieve optimal outcomes.
    • Provide drug and clinical information to other healthcare professionals.
    • Develop and communicate specific pharmacotherapeutic plans for individual patients.
    • Provide accurate and timely medication reconciliation and updates of medication lists.
    • Identify and intervene appropriately to resolve potential medication problems for all assigned patients.
    • Evaluate and provide recommendations on pharmacokinetic or nutrition consults as applicable.
    • Participate in discharge planning for assigned inpatients.
    • Proactively assess patients' financial status and address barriers.
    • Participate in and guide medical staff to comply with institutional standards, initiatives, and guidelines.
  • Education and Training
    • Provide effective medication and practice-related education or counseling to patients and caregivers.
    • Provide accurate and timely medication, treatment guidelines, formulary, and other pertinent information to members of the healthcare team.
    • Develop and maintain a desire and aptitude for teaching, including mastery of the four preceptor roles when teaching clinical problem-solving to trainees.
    • Develop and maintain the ability to provide comprehensive and constructive criteria-based feedback and evaluation of resident/student performance.
    • Serve as an effective preceptor for resident, student, and other trainee clinical learning experiences.
    • Precept other trainee-required experiences, including oncology forum, research projects, grand rounds, MUEs, etc.
    • Collaborate with student and residency leadership to coordinate individual learning experiences.
    • Attend and actively participate in trainee learning activities, including grand rounds, student presentations, pharmacy pearls, journal club, grand rounds workshops, etc.
    • Provide didactic lectures, in-services, shared student instruction, or attend case conference presentations to trainees.
    • Model professional behavior to orient, mentor, and train new clinical staff on patient care services, policy, and procedures.
  • Clinical Support
    • Use technology to facilitate safe and efficient delivery and communication of patient care.
    • Participate in medication-related processes to assess, evaluate, maintain, and improve the delivery of patient care.
    • Participate in institutional initiatives to develop, improve, validate, and implement research and standard of patient care tools.
    • Adhere to regulatory/compliance policies and procedures that govern the profession and scope of practice.
    • Complete pharmacy protocol reviews to facilitate the IRB submission process.
    • Analyze their own practice, experience, medical literature, and other information resources for self-improvement.
    • Obtain and maintain required and preferred licenses, certifications, training, and continuing education prior to deadlines.
    • Document and review clinical activity/interventions appropriately.
    • Use professional judgment and language to report safety events.
    • Model service excellence.
    • Attend and actively participate in committees/meetings.
    • Demonstrate effective oral and written communication skills.
    • Align patient care activities and project work with departmental and institutional policies, business model/needs, goals, and vision.
    Requirements
    • Pharm.D. degree.
    • Texas State Board of Pharmacy Pharmacist license within 90 days of job entry.
    • Board Certification in an appropriate specialty area.
    • Pharmacy Practice Residency (PGY 1) and Residency in the area of specialization (PGY 2).
    • Experience as a clinical pharmacist in the area of specialization may substitute for the required residency on a one-to-one basis.
