Job Description

We are seeking an experienced Oncology Nurse Navigator I to join our team. As a key member of our interdisciplinary team, you will play a crucial role in enhancing the patient experience by providing seamless navigation through the cancer continuum.

Responsibilities:
  • Coordinate patient care through the cancer continuum by assisting with timely access to care, coordinating/facilitating appointments and services, and encouraging adherence to treatment plans, protocols, and follow-up care.
  • Assess patient current and future needs, provide patient access to resources (internal and external), and make appropriate referrals as needed.
  • Provide education and information to the patient and family related to the cancer healthcare system, multidisciplinary team member roles, plan of care, and available resources.
  • Establish and maintain close collaboration with physicians and multidisciplinary team members to ensure seamless patient care and care coordination.
  • Track navigation program metrics, quality indicators, document patient interactions and progression, and communicate navigation activities to physicians, cancer care team, and department.
Required Skills and Qualifications

To be successful in this role, you must have:

  • An ASN degree; BSN preferred.
  • A minimum of three years of clinical experience.
  • Oncology experience is preferred.
  • ONS certification is preferred or completed within one year of hire.
  • Maintains license as an RN in the state of Florida.
  • Maintains current BLS/healthcare provider certification.
