Oncology Care Coordinator Lead

As a dedicated oncology care coordinator, you will play a vital role in ensuring the continuity and consistency of care for patients across various settings. You will work closely with clinical teams to identify patients who would benefit from care coordination services and assess their risk factors and needs. Your primary goal is to facilitate comprehensive discharge planning and ensure timely follow-up care.

Your Responsibilities
As an oncology care coordinator, your key responsibilities will include:
• Taking the lead in ensuring continuity and consistency of care across the continuum
• Collaborating with Patient and Family Counselors/Social Workers and Utilization Review Nurses to engage patients and families in care planning
• Monitoring progress towards discharge plans and adjusting as needed
• Educating patients and families about evidence-based standards of care and self-management strategies
• Identifying support needs and developing action plans to address them
• Ensuring patients have access to necessary prescriptions, durable medical equipment, and other services
• Contributing to problem-solving within the team through communication, collaboration, and data collection
Requirements
To be successful in this role, you will need to possess:
• A Bachelor of Science in Nursing (BSN) degree
• Current licensure as an RN in the State of Florida
• Current BLS/healthcare provider certification
• Three years of direct clinical experience with an emphasis on oncology care
