Key Responsibilities:- Individualized Care Plans: Collaborate in developing tailored patient care plans, identifying needs and anticipated outcomes in compliance with Federal and State regulations.
- Interdisciplinary Collaboration: Engage in the nursing center's interdisciplinary Resident Assessment Instrument (RAI) process, ensuring adherence to RAI guidelines for MDS completion, encoding, and electronic submission.
- Patient Condition Monitoring: Identify and report changes in patient conditions, coordinating with the interdisciplinary team to schedule and complete necessary assessments.
- Staff Training: Facilitate training for new hires on Activities of Daily Living and provide ongoing education for existing staff.
- Regulatory Knowledge: Attend educational programs to stay updated on current RAI and Prospective Payment System regulations.
