Key Responsibilities of the Licensed Practical Nurse (LPN):
- Provide direct patient care utilizing advanced medical technology in partnership with the RN Case Manager and interdisciplinary team.
- Establish meaningful connections with patients to enhance their quality of life.
- Maintain regular communication with the care team to ensure optimal patient care.
- Educate patients and their families on effective self-care practices.
- Adhere to physician directives and established treatment protocols.
- Collaborate within an interdisciplinary team to deliver timely and effective patient services.
- Document patient care and relevant data accurately.
- Aim for positive clinical outcomes and high-quality care standards.
- Participate in weekend and holiday duty rotations.
- Engage in interdisciplinary case discussions.
Qualifications:
- Graduate from an accredited nursing program.
- Licensed as an LPN in Rhode Island and Massachusetts, with pediatric experience.
- At least 1 year of clinical practice experience.
- Strong clinical and communication abilities.
- Must obtain dual licensure in Rhode Island and Massachusetts within six months.
- Preferred experience in Hospice or Home Care settings.
- Background in care coordination is advantageous.
