Job Title: Hospice Registered Nurse

HopeHealth is seeking a compassionate and skilled Registered Nurse to join our Hospice Care Team. As a Hospice Registered Nurse, you will provide end-of-life care to patients in their homes and long-term care facilities.

Key Responsibilities:
  • Conduct skilled assessments to determine patient eligibility for hospice services
  • Complete admissions for eligible patients in compliance with Medicare/Medicaid and state regulations
  • Develop individualized plans of care, including documenting Local Coverage Determination
  • Participate in weekly interdisciplinary group meetings to review patients on service
  • Document in EMR at point of care and meet department expectations for timely completion of documentation
Requirements:
  • RN required; BSN preferred
  • Licensed as a Registered Nurse in Rhode Island and Massachusetts
  • Minimum of 1-3 years of clinical experience
  • Excellent nursing assessment, clinical, and communication skills
