The Director of Case Management is responsible for providing comprehensive leadership and strategic oversight for Case Management operations. This role is essential in fostering and ensuring the progression of Coordinated Care throughout the healthcare continuum. The Director holds ultimate responsibility, authority, and accountability for the Case Management Department, overseeing all daily operational aspects to achieve both immediate and long-term objectives.
Key Qualifications:
- Active Registered Nurse license is mandatory
- Bachelor's Degree in Nursing is essential
- Proven experience as a Director of Case Management in an Acute Care setting is required
- Certification in Case Management (CCM or ACM) is preferred
