As a Case Manager at Universal Health Services, you will play a vital role in coordinating the delivery of healthcare services to our inpatient hospital population and their families. This position requires a strong understanding of healthcare operations and a passion for delivering quality patient care.
Key Responsibilities
- Coordinate the delivery of healthcare services to inpatient hospital population and their families
- Develop and implement effective discharge plans to ensure seamless transitions of care
- Collaborate with interdisciplinary teams to identify and address patient needs
- Conduct thorough assessments to determine patient eligibility for various programs and services
- Develop and maintain relationships with community resources and providers to ensure access to necessary services
Requirements
- Minimum 5 years of acute care clinical experience
- Minimum 3 years of clinical case management/utilization review experience
- Current LVN California license and BLS certification
- CPI certification within 30 days of hire
