We are seeking a highly skilled Oncology Nurse Navigator to join our team at Christus Health. The successful candidate will be responsible for coordinating patient care in a timely and efficient manner, identifying and eliminating barriers to care.
Key Responsibilities- Coordinate patient care across the continuum, ensuring seamless transitions and optimal outcomes.
- Assess patient needs and develop individualized care plans to address physical, psychological, and social needs.
- Provide expert communication and guidance to patients and families, addressing health care issues and concerns.
- Collaborate with healthcare providers and staff to ensure comprehensive and coordinated care.
- Participate in quality improvement initiatives and data collection to enhance patient outcomes and care coordination.
- Registered Nurse, BSN in Nursing, preferred.
- Current New Mexico Nursing License and BLS certifications issued by the American Heart Association required.
- Minimum of 5 years of experience in a healthcare setting, with related experience in quality, care coordination, and oncology preferred.
This role requires the ability to work in a fast-paced environment, with multiple settings and variable unpredictable situations. The successful candidate must be able to move around settings of care, with close work, good vision, and dexterity to write and use a computer keyboard.
