Youth Health Advocate

We are seeking an experienced Youth Health Advocate to join our team at Seven Hills Foundation. In this critical role, you will work closely with nursing staff to coordinate care for students with chronic health conditions and promote a healing environment in our residential treatment program for male youth. You will be responsible for developing and implementing individualized care plans, collaborating with our interdisciplinary team, and ensuring seamless communication and coordination of care.
Key Responsibilities
* Develop and implement individualized care plans in collaboration with nursing staff and other interdisciplinary team members
* Coordinate care for students with chronic health conditions, including managing medication regimens and monitoring health status
* Collaborate with our interdisciplinary team to ensure seamless communication and coordination of care
* Promote a healing environment through education and support to students and their families
* Participate in regular team meetings to discuss patient progress and adjust care plans as needed
Requirements
* Bachelor's degree in Nursing or related field
* Minimum of 2 years of experience in a similar role or as a registered nurse
* Excellent communication and organizational skills
* Ability to work effectively in a fast-paced environment
* Strong attention to detail and ability to maintain accurate records
Salary Information
This full-time position comes with a competitive salary range of $63,000 - $83,000 per year, depending on experience.
