Healthcare Navigator

The Care Coordinator is responsible for providing social support and connecting patients with community resources to ensure continuity of care. This role involves identifying patient gaps in care, collaborating with the healthcare team, and communicating with patients to ensure they receive necessary care.
Key Responsibilities- Use reports and health information technology to identify patient gaps in care
- Collaborate with the healthcare team to confirm patient needs
- Communicate with patients about needed care and follow up to ensure they receive it
- Develop workflows to ensure population management activities are aligned and coordinated
- Active RN in the State of Oregon
- Active Community Healthcare Worker (CHW) certification or obtain it within one year of hire
- One (1) year experience in community outreach, health education, or a role representing or supporting local community agencies
- Preferred: Three (3) or more years experience in community outreach, health education, or a role representing or supporting local community agencies
- Experience in community outreach services providing health information, advocacy, social support, and assistance
- Ability to communicate effectively and relate to individuals from various economic, social, and cultural backgrounds
- Ability to use a range of outreach methods to engage individuals and groups in diverse settings
- Demonstrated ability to connect patients to resources within the community/population areas served
