Job Title: Patient Care Coordinator, RN

Job Summary:
We are seeking a skilled Patient Care Coordinator, RN to join our team at The Children's Clinic. As a key member of our healthcare team, you will play a vital role in providing social support to patients and their families, helping them navigate and access community services and resources. Your expertise will also be instrumental in facilitating continuity of care by providing client follow-up and ensuring patients receive the necessary care.

Key Responsibilities:
  • Identify patient and population gaps in care using reports, health information technology, and registries.
  • Collaborate with the healthcare team/provider to confirm needs and communicate with patients about required care.
  • Develop workflows and ensure population management activities across the organization are aligned and coordinated.
  • Conduct activities directly and collaborate with others to achieve optimal patient outcomes.

Requirements:
  • Active RN in the State of Oregon.
  • Must have active Community Healthcare Worker (CHW) certification or obtain it within one year of hire.
  • One (1) year experience in community outreach, health education, or in a role representing or supporting local community agencies.
  • Preferred: Three (3) or more years experience in community outreach, health education, or in a role representing or supporting local community agencies.

Preferred Skills:
  • Experience in community outreach services providing health information, advocacy, social support, and assistance.
  • Ability to communicate effectively and relate to individuals from various economic, social, and cultural backgrounds.
  • Ability to use a range of outreach methods to engage individuals and groups in diverse settings.
  • Demonstrated ability to connect patients to the resources within the community/population areas served.
