Job Summary

We are seeking a highly skilled RN Stroke Navigator to join our team at Providence Oregon Regional Shared Services. As a RN Stroke Navigator, you will play a critical role in providing exceptional patient care and coordination between patients, primary care physicians, neurologists, and other specialists.

Key Responsibilities
  • Conduct early post-discharge screening for medication compliance, healthy lifestyle, depression, medical complications, and caregiver burden.
  • Provide coordination and support to patients and their families throughout the care continuum.
  • Collaborate with healthcare teams to ensure seamless transitions of care and optimal patient outcomes.
  • Advocate for patients and their families, addressing their needs and concerns in a timely and compassionate manner.
Requirements
  • Bachelor's Degree in Nursing or obtained within 3 years.
  • Oregon Registered Nurse License upon hire.
  • National Provider BLS - American Heart Association upon hire.
  • 5 years of related neurological clinical experience.
Preferred Qualifications
  • Master's Degree in Nursing.
  • NIHSS Certification upon hire.
  • Stroke Certified Registered Nurse upon hire.
  • American Association of Neuroscience Nursing membership and certification upon hire.
