VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our Home Health team in North Arlington, New Jersey.

As a Registered Nurse in our Home Health team, you will be responsible for providing high-quality, patient-centered care to individuals in their homes, promoting independence, and improving their overall well-being.
Responsibilities:- Assess and evaluate patients' needs, developing personalized care plans to meet their unique requirements.
- Provide skilled nursing care, including wound care, medication management, and education on self-care techniques.
- Collaborate with interdisciplinary teams to ensure comprehensive care and coordinate services.
- Document patient progress, updating care plans and outcomes as necessary.
- Identify teaching needs and develop strategies to educate patients and families on health management.
- Monitor quality and appropriateness of services provided, adhering to agency policies and procedures.
- Current RN licensure in New Jersey.
- 1+ year of supervised clinical experience as a registered nurse.
- Excellent verbal and written communication skills.
- Strong computer skills and ability to navigate electronic medical records.
- Excellent organizational and interpersonal skills.
- Valid driver's license and reliable transportation.
