Job Summary:We are looking for a dedicated and experienced Registered Nurse to provide high-quality patient care in the comfort of their own homes. As a member of our Home Health team, you will work closely with patients, families, and healthcare providers to develop and implement individualized care plans that promote optimal health and well-being.
Responsibilities:- Assess and evaluate patient needs, developing and implementing effective care plans to address complex health conditions and co-morbidities.
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on disease management and self-care.
- Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between healthcare settings.
- Document patient progress and outcomes, maintaining accurate and comprehensive records.
- Identify teaching needs and develop strategies to educate patients and families on health management and disease prevention.
- Current RN licensure in New Jersey.
- 1+ year of supervised clinical experience as a Registered Nurse.
- Excellent verbal and written communication skills.
- Strong computer skills and proficiency in electronic medical records.
- Ability to work independently and as part of a team.
