Job Title: Home Health Registered Nurse

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our Home Health team in North Arlington, New Jersey.

Job Summary:

We are looking for a dedicated and experienced Registered Nurse to provide high-quality patient care in the comfort of their own homes. As a Home Health Registered Nurse, you will be responsible for assessing patient needs, developing individualized care plans, and delivering evidence-based care to patients with multiple disease processes and co-morbidities.

Responsibilities:
  • Assess and evaluate patient needs, developing and implementing individualized care plans to achieve optimal patient outcomes.
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on disease management and self-care.
  • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between healthcare settings.
  • Document patient progress, updates, and outcomes in a timely and accurate manner.
  • Identify teaching needs and develop strategies to educate patients and families on disease management, self-care, and health promotion.
  • Monitor quality and appropriateness of services provided, ensuring compliance with regulatory requirements and VNA Health Group policies.
Requirements:
  • Active New Jersey RN License
  • Bachelor's degree in Nursing preferred
  • 1+ year of supervised clinical experience as a registered nurse
  • Excellent verbal and written communication skills
  • Strong computer skills
  • Excellent organizational and interpersonal skills
  • Car and valid driver's license
