Job Title: Registered Nurse Home Health

VNA Health Group is seeking a skilled Registered Nurse to provide high-quality home health care to patients in North Arlington, New Jersey.

Job Summary:

The successful candidate will be responsible for assessing and evaluating patient needs, developing and implementing individualized care plans, and providing ongoing support and education to patients and their families.

Responsibilities:
  • Assess and evaluate patient needs, identifying areas for improvement and developing targeted interventions.
  • Develop and implement individualized care plans, incorporating patient goals and preferences.
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination.
  • Document patient progress and outcomes, maintaining accurate and timely records.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
Requirements:
  • Current RN licensure in New Jersey.
  • 1+ year of experience in home health care or a related field.
  • Excellent communication and interpersonal skills.
  • Strong organizational and time management skills.
  • Ability to work independently and as part of a team.
