Patient Care Facilitator

First Choice Community Healthcare INC is seeking a highly skilled Patient Care Facilitator to join our team. As a Patient Care Facilitator, you will play a vital role in ensuring the delivery of high-quality patient care and services.
Job SummaryThe Patient Care Facilitator will be responsible for coordinating patient care activities, facilitating communication between healthcare providers and patients, and ensuring that patients receive the necessary support and resources to achieve optimal health outcomes.
Key Responsibilities- Coordinate patient care activities, including scheduling appointments, managing patient records, and communicating with healthcare providers.
- Facilitate communication between patients, healthcare providers, and other stakeholders to ensure seamless care coordination.
- Identify and address patient needs, providing support and resources to ensure optimal health outcomes.
- Collaborate with healthcare providers to develop and implement patient care plans.
- Monitor patient progress, identifying areas for improvement and implementing changes as needed.
- Provide education and support to patients and their families to promote health literacy and self-management.
- Participate in quality improvement initiatives to enhance patient care and services.
To be successful in this role, you will need:
- A medical assistant training or comparable directly related clinical experience.
- Direct work experience in a healthcare setting, with knowledge of ambulatory clinical operations and available patient support programs.
- Knowledge of medical terminology and patient intake procedures.
- Bi-lingual English/Spanish is highly preferred.
This role requires:
- Good dexterity to operate personal computers and office equipment.
- Occasional lifting and carrying related to office duties.
- Capable of using office machines and personal computers for word processing, data entry, and spreadsheet applications.
- Must be able to read a computer monitor and outputs accurately.
- Must be able to communicate clearly and accurately for work and safety compliance.
This is a full-time position, working regularly scheduled Monday-Friday, although weekends may be required to meet deadlines.
