Job Title: Patient Care Facilitator

First Choice Community Healthcare INC is seeking a highly skilled Patient Care Facilitator to join our team. As a Patient Care Facilitator, you will play a vital role in ensuring the seamless coordination of patient care, working closely with our healthcare delivery teams and outside entities to provide exceptional patient outcomes.

Key Responsibilities:
  • Facilitate patient access to community resources, educational programs, and healthcare services, ensuring timely and effective care.
  • Participate in the development and implementation of healthcare quality improvement initiatives, driving population health and patient satisfaction.
  • Conduct patient chart reviews, pre-visit planning, and outreach activities to enhance visit efficiency and effectiveness.
  • Communicate with patients, providers, and staff to coordinate care, resolve issues, and ensure seamless transitions.
  • Document activities and interventions within patient charts, maintaining accurate and up-to-date records.
Requirements:
  • Medical assistant training or comparable clinical experience.
  • Direct work experience in a healthcare setting, with knowledge of ambulatory clinical operations and patient support programs.
  • Knowledge of medical terminology, patient intake, and charting procedures.
  • Bi-lingual English/Spanish is highly preferred.
