As a Clinical Pharmacist, you will be responsible for developing and participating in collaborative partnerships with medical staff, reviewing and monitoring outpatient medication regimens, and performing dispensing functions in accordance with established policies and directives.
Key Responsibilities- Assess patients' response to drug therapy and plan drug therapy based on physician-established diagnoses.
- Order and assess laboratory tests necessary to evaluate drug therapy effects and therapeutic outcomes.
- Analyze patterns and trends to determine appropriate, effective, and economical drug therapy management and recommend alternatives.
- Modify dosages, methods, or significantly alter regimens without prior approval of physicians, subject to post-action reviews.
- Provide clinical pharmacy services to selected clinics, including drug therapy management, patient education, therapeutic drug monitoring, staff education, and solicited drug therapy consults.
- Document clinical pharmacy services and therapeutic consults, including economic impact, on a continuous basis and prepare related reports.
- Participate in quality assurance activities, such as drug utilization evaluations and measures of clinical pharmacists' impact on drug therapy.
- Possess a six-year Doctor of Pharmacy (Pharm D) degree from a College of Pharmacy accredited by the American Council on Pharmaceutical Education (ACPE).
- Possess a minimum of two years post-graduate experience as a clinical pharmacist within the last three years.
- Possess a current, unrestricted license to practice pharmacy in any one of the United States of America, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands.
- Possess a current Drug Enforcement Administration (DEA) Registration certificate and a National Provider Identifier (NPI).
- Hold current Basic Life Support (BLS) hands-on certification through the American Heart Association or American Red Cross.
- Be a U.S. citizen.
- Be able to read, write, and speak English well enough to effectively communicate.
- Be physically capable of standing and/or sitting for extended periods of time and performing all services required under the contract.
Shifts will be Monday through Friday, with normal shifts being 8 hours between 6:00 am and 8:00 pm, including a 30 or 60-minute break. Shifts will be set by the scheduling officer as soon as possible.
