Job Summary

We are seeking an experienced Registered Nurse (RN) to join our team as a Weekend Clinical Coordinator. This role provides a supportive role to the clinical management team and performs a variety of duties in the coordination of the delivery of care to patients and families in their homes.

Key Responsibilities
  • Triage calls related to patient care, including physician orders, staff issues, and other clinical issues.
  • Provide effective coordination of clinical services, consistent with the objectives of the agency.
  • Work closely with clinical staff to provide necessary services for patients and families in a timely manner, including the provision of direct patient care.
  • Communicate regularly with Clinical Managers and the Clinical Director on issues related to clinical practice or performance.
  • Assist with individual caseload reviews or other audits as assigned by the manager or Clinical Director.
  • Assist with orientation and preceptor of clinical staff.
  • Evaluate performance of each individual in relation to established standards in collaboration with the clinical manager.
  • Determine staff competency in a fair and consistent manner and provide direction as necessary.
  • Work collaboratively with all disciplines to facilitate productivity, cooperation, and efficient operations.
  • Assist in clinical manager's and scheduling coordinator in appropriate coverage based on census, skill mix, geographic location, patient need, and clinical capacity.
  • Triage calls from physicians and follow through on required actions, including communication to field staff, changes in orders, scheduling PRN visits, and follow-up as a clinical resource to clinical staff if needed.
  • Provide documentation that is concise, accurate, and informative and supports reimbursement.
  • Practice in accordance with standards of care, policies, procedures, and scope of practice, demonstrating an awareness of regulatory guidelines.
  • Ensure compliance within guidelines set forth by regulatory agencies (DPH, ERISA, etc.) and demonstrate compliance with Tufts Medicine Care at Home policies and procedures.
Requirements
  • Registered Nurse (RN)
  • Three (3) years of community health experience required or combination of at least one year of acute medical/surgical nursing experience and 2 years of home health experience required.
