Dietitian Clinical Specialist

In the role of Registered Dietitian, you will be responsible for implementing the Nutrition Care Process (NCP) in our organization. This involves conducting comprehensive nutrition assessments, identifying malnutrition and morbid obesity, evaluating food insecurity, and developing individualized goals and interventions for participants with complex medical conditions.
Key Responsibilities- Conduct comprehensive nutrition assessments at the time of enrollment and semi-annually thereafter according to CMS guidelines.
- Utilize screening tools and apply them effectively in your work.
- Assess nutritional status using medical history, laboratory data, medications, anthropometric measurements, and diet history.
- Perform nutrition-focused physical exams during assessments to determine risk and/or degree of malnutrition when indicated.
- Evaluate psychosocial, functional, and behavioral factors related to food access, selection, preparation; physical activity; and understanding of health condition.
- Evaluate participant and family's knowledge, readiness to make changes towards positive outcomes, and motivation for changing behaviors.
- Collaborate with other IDT members or other healthcare professionals when indicated.
- Finding patterns and possible causes for the nutrition problem.
- Documenting the nutrition diagnostic statement in PES (problem, etiology, and signs/symptoms) and providing supporting evidence within the documentation.
- Formulating and determining individualized interventions focused on the etiology of the problem.
- Assessing educational needs and providing individualized nutrition education and counseling based on the individual, including nutrient requirements, knowledge and abilities, and medical, economic, and social situation.
- Coordinating nutrition plan of care with the IDT team and other healthcare professionals when indicated.
- Utilizing monitoring and evaluation techniques that consider participants' nutrition needs as well as cultural, religious, and ethnic concerns.
- Evaluating the effectiveness and measures progress toward outcomes of nutrition interventions.
- Monitoring participants to be identified at high nutritional risk and provides timely follow-up based on department criteria and clinical judgment.
The estimated salary for this position is $85,000 - $110,000 per year, depending on experience.
