Role Objective:
The primary aim of this position is to organize and manage medical, dental, and counseling appointments for the individuals supported by EPI Inc.
Key Responsibilities:
- Ensure the health and welfare of the individuals under EPI Inc.'s care.
- Organize and manage all medical appointments, including urgent visits, and provide transportation and accompaniment to these appointments.
- Communicate appointment schedules and outcomes to the relevant support networks.
- Ensure that medical providers document appointment results and recommendations accurately, and distribute copies of consults to the necessary personnel.
- Notify the Registered Nurse and supervisor of any required updates to individual plans.
- Assist in the procurement and maintenance of necessary adaptive equipment and supplies.
- Report any policy breaches to the appropriate authorities.
- Promote a safe working environment.
Education and Experience Requirements:
- High school diploma or equivalent, with at least one year of experience in the human services sector.
Required Qualifications:
- Possession of a valid driver's license in New York State; a vehicle is necessary for job performance.
- Must obtain medication certification (if mandated by funding sources) and complete all required training within 90 days of employment.
- Proficiency in reading, writing, and speaking English is essential.
- Maintain current certifications for all mandatory training.
- Must achieve van certification.
Physical Requirements:
The physical demands outlined here are representative of those that must be met by an employee to successfully fulfill the essential functions of this role. Reasonable accommodations may be made to enable individuals with disabilities to perform these functions.
- Ability to lift, transfer, and move at least 50 pounds.
- Capability to communicate, hear, stand for extended periods, walk, sit, twist, bend, and use hands and arms effectively.
- Specific vision abilities required for this role include close vision and the ability to adjust focus.
- Employees must be trained in and capable of performing Strategies for Crisis Intervention and Prevention - Revised (SCIP-R).
- Must be able to perform CPR and first aid.
This position offers a competitive hourly wage.
