**Job Summary:**

VIVA HEALTH is seeking a dedicated Care Coordinator to join our team in Birmingham, AL. As a key member of our healthcare team, you will work closely with nurses and social workers to provide non-clinical services for Medicaid recipients.

About the Role:

The Care Coordinator will analyze complex situations, identifying barriers to medical compliance and developing effective care plans. This role requires strong communication skills, analytical thinking, and the ability to work independently in an autonomous setting.

Responsibilities:**

  • Provide non-clinical services for Medicaid recipients
  • Analyze complex situations to identify barriers to medical compliance
  • Develop effective care plans to improve medical compliance and health outcomes

Requirements:**

  • Licensed BSW, Licensed BSN, or Licensed ADN
  • Experience in case management, human services, public health, or working with underinsured populations
  • Valid driver's license and willingness to submit to vaccine testing and screening

Salary and Benefits:

The estimated salary for this role is $45,000 - $60,000 per year, depending on experience. VIVA HEALTH offers a comprehensive benefits package, including tuition reimbursement, paid parental leave, and paid day off for community service.
