Position Overview

We are looking for a dedicated Community Support Specialist to become a part of our RISE Plus Program team. In this role, you will report directly to the RISE Plus Program Manager and be instrumental in delivering comprehensive case management services to clients involved in the RISE Plus Transitional Housing and Rapid Rehousing Program. Your work will primarily focus on clients living in scattered-site units within the District of Columbia.

Your responsibilities will place you at the forefront of direct assistance and intervention, aimed at empowering clients as they navigate their paths toward safety, stability, and independence. A key aspect of your role will involve conducting in-person case management sessions, ensuring that clients receive support with the highest level of care and compassion. You will meet with clients at least twice a month, collaborating closely to develop detailed service plans and monthly financial budgets. Through thorough assessments, you will identify the unique strengths and needs of each individual or family, making appropriate referrals to address their social, environmental, medical, and emotional challenges.

This position offers a hybrid work environment, requiring you to be in the office 1-2 times a week while also engaging with clients in the community.

About My Sister's Place

My Sister's Place is a dedicated community of staff, volunteers, and advocates focused on eliminating domestic violence. As the oldest domestic violence shelter in DC, we have been a cornerstone in addressing this critical public health issue since 1979. Our organization uniquely provides a continuum of care that ranges from emergency shelter to transitional and permanent housing. Our skilled team of case managers and residential counselors deliver clinical counseling, case management, and a variety of services designed to empower survivors to recover and thrive. Additionally, we offer training, case consultation, and advocacy to engage communities in preventing violence and abuse.

Key Responsibilities

Case Management

  • Deliver intensive case management services to individuals and families impacted by domestic violence and other crimes through in-person meetings twice a month, supplemented by additional communication via email, text, or phone.
  • Develop detailed service plans, safety plans, and financial budgets with each client, reviewing them on a monthly basis.
  • Build and strengthen relationships with partners and stakeholders, including landlords.
  • Assess the strengths and needs of individuals and families in relation to their overall intervention plan.
  • Make suitable referrals to address social, environmental, medical, and emotional challenges.
  • Collaborate with local service providers to meet individual client needs.
  • Provide support during crisis situations and intervene to resolve immediate issues.
  • Assist clients in accessing housing, mental health services, employment opportunities, financial assistance, and other necessary resources.

Data Management and Reporting

  • Maintain accurate and up-to-date records for documenting case activities, including electronic case management data entry.
  • Prepare various reports, including monthly updates for the Program Manager.
  • Participate in and help facilitate workshops on financial literacy, housing opportunities, and other relevant topics as needed.
  • Assist in the training and supervision of volunteers as required.

Qualifications

  • Minimum of two years of case management experience is required.
  • Experience working with individuals who have faced trauma is a plus.
  • Comprehensive understanding of human behavior dynamics.
  • In-depth knowledge of domestic violence dynamics.
  • Familiarity with DC social services resources and processes.
  • Experience with HMIS (Homeless Management Information System) and CAHP (Coordinated Assessment and Housing Placement) System is preferred.
  • A valid driver's license is required.
  • Possession of a valid DC social work license is a bonus.
  • Bilingual candidates in English and Spanish are encouraged to apply.
