Job Title: Transitional Living Program Case Manager

Job Summary:

Lighthouse is seeking a highly motivated and compassionate Transitional Living Program Case Manager to join our team. The successful candidate will provide case management services to youth enrolled in our Transitional Living Program, helping them achieve their goals and regain long-term stability.

Responsibilities:

  • Provide case management services to youth enrolled in the Transitional Living Program, including goal planning, advocacy, and support.
  • Conduct screenings and intakes for the program via the Homeless Management Information System (HMIS).
  • Maintain a caseload of up to 20 active households and provide regular case management services, including group notes, case notes, and service transactions in HMIS.
  • Assist youth in developing and implementing an Individual Plan of Service (IPOS) and connect them with resources based on their goals.
  • Provide education and information to the community regarding the Transitional Living Program and housing services.
  • Collaborate with other service providers to ensure a coordinated and collaborative approach to service delivery.
  • Perform other job-related duties as assigned.

Requirements:

  • Lived or professional experience working with people experiencing homelessness or extreme levels of poverty in the US.
  • Associates degree or equivalent experience.
  • 1-2 years' experience in a role directly or indirectly related to human/social services and/or case management.
  • Reliable transportation and valid driver's license.
  • Knowledge of Microsoft Office Suite and Google Apps.
  • Flexibility and ability to prioritize tasks in a rapidly changing environment.
  • Ability to work with a challenging and diverse population.

Preferred Qualifications:

  • BSW from a 4-year accredited college or university or master's degree in a related field.
  • 2+ years' experience in a role directly or indirectly related to human/social services and/or case management.
  • Previous experience providing homeless recovery services to youth.
  • Knowledge of the social services landscape in Oakland County and surrounding areas, trauma-informed principles and values, Positive Youth Development, behavioral management, de-escalation techniques, basic intervention skills, and homeless recovery services.

Working Conditions:

The incumbent typically works in an office environment and uses a computer, telephone, and other office equipment as needed to perform duties. The noise level in the work environment is typical of that of an office. Incumbent may encounter frequent interruptions throughout the workday.

Physical Demands:

The employee is regularly required to sit, talk, or hear; frequently required to use repetitive hand motion, handle or feel, and to stand, walk, reach, bend, or lift up to 20 pounds. Contact with the public in office environments may risk exposure to irrational/hostile behavior, contagious diseases, or contact with domestic animals.
