Scope of Work

As a Clinical Nurse Specialist, you will plan and prioritize care for individuals and populations, focusing on strategies that promote optimal health. You will demonstrate expertise in nursing care and management, seeking to improve patient, family, and health systems outcomes through educational concepts and preventive care.

  • Assess patient needs, level of intensity, insurance benefits, and other resources to develop a comprehensive care plan.
  • Collaborate with healthcare teams to coordinate services and promote continuity of care.
  • Communicate with patients, families, and providers to ensure seamless care transitions.
  • Measure patient outcomes, interpret reports, and analyze data trends to inform care decisions.
  • Document patient data, plan, interventions, and outcomes according to department guidelines.
