Job Summary

We are seeking a highly skilled and experienced Manager of Care Management to join our team at Corewell Health William Beaumont University Hospital. As a key member of our Care Management Department, you will be responsible for leading a dynamic team of healthcare professionals in delivering personalized, high-quality care to our patients.

Key Responsibilities
  • Lead a team of up to thirty healthcare professionals in delivering care management services to patients.
  • Develop and implement effective care management strategies to improve patient outcomes and reduce healthcare costs.
  • Collaborate with multiple disciplines to expedite care coordination functions and ensure seamless patient care.
  • Interview, hire, discipline, evaluate, and manage direct reports to ensure high-performing team members.
  • Provide input into department policies, procedures, objectives, and budget to promote organizational and departmental objectives.
  • Problem-solve and intervene with physician, nursing staff, and other ancillary staff to facilitate patient care and resolve barriers to care management functions.
Requirements
  • Bachelor's degree in nursing (BSN) or Master's degree in social work (MSW) required.
  • Five years of relevant clinical experience and three years of acute care management experience required.
  • Excellent interpersonal relations, communication skills, and decision-making skills required.
  • MI RN license or MI LMSW license required.
