Job Title: Licensed Practical Nurse

At HRA, we are seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team. As a key member of our healthcare staff, you will provide direct and individualized nursing care to our residents, ensuring their physical and emotional well-being.

Key Responsibilities:
  • Deliver high-quality nursing care to residents, adhering to established standards and protocols.
  • Implement care plans developed by physicians and the Director of Resident Care (DRC).
  • Document resident care accurately and in a timely manner.
  • Assess and provide resident and family/caregiver information pertinent to diagnosis or care plan.
  • Administer medication as prescribed by physicians, documenting properly on the Medication Administration Record (MAR).
  • Assemble and maintain resident charts and records, ensuring all necessary forms are complete and accurate.
  • Prepare incident/accident reports as needed, notifying physicians and families accordingly.
  • Maintain a safe and healthy working environment, adhering to clinical standards and procedures.
  • Assist residents in learning self-care activities.
  • Organize and monitor resources necessary for resident care.
  • Recognize emergencies and life-threatening situations, initiating appropriate action.
  • Maintain confidentiality and resident rights regarding personal information.
  • Perform other duties as assigned by supervisors.
Requirements:
  • Graduation from an accredited LPN/LVN program and current registration in the state of employment.
  • Valid Continuing Education credits as required by the state.
  • Training in Alzheimer and Dementia with a focus on Validation Therapy.
  • Minimum one year of nursing experience in a healthcare facility.
  • Experience in long-term care and/or geriatric nursing preferred.
  • Experience in emergency care and first aid.
  • Effective communication skills with all levels of management, associates, residents, family members, and outside contacts.
  • Experience with Microsoft Office and Outlook software preferred.
  • Able to make independent decisions and seek out new methods to incorporate into existing practices.
  • Passion for working with and around senior citizens.
