Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at HRA. As a key member of our care team, you will provide high-quality, individualized nursing care to our residents, ensuring their physical and emotional well-being.

Key Responsibilities
  • Deliver direct care to residents, including administering medications, providing wound care, and assisting with daily living activities.
  • Develop and implement individualized care plans in collaboration with the Director of Resident Care and other healthcare professionals.
  • Conduct thorough assessments of residents' physical and emotional needs, documenting findings and progress in resident charts.
  • Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination.
  • Maintain accurate and timely documentation of resident care, including medication administration records and incident reports.
Requirements
  • Graduate of an accredited LPN/LVN program and current registration in the state of employment.
  • Minimum one year of experience in a healthcare facility, preferably in long-term care or geriatric nursing.
  • Training in Alzheimer's and dementia care, with a focus on validation therapy.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Proficiency in Microsoft Office and Outlook software.
