Job Title: Recovery Manager

CareStar, Inc. is seeking a skilled Recovery Manager to join our team in Miami County and surrounding areas. As a key member of our Operations team, you will play a vital role in conducting eligibility evaluations and creating person-centered service plans for individuals applying for specific programs.

Key Responsibilities:
  • Maintain knowledge of Medicaid, the behavioral health system, and community resources to ensure effective service coordination.
  • Conduct face-to-face eligibility evaluations, including the administration of the Adult Needs and Strengths Assessments, to verify residency and qualifying behavioral health diagnoses.
  • Develop, monitor, and update individualized care plans to ensure the individual's needs, preferences, health, and welfare are met.
  • Coordinate and authorize specialized recovery services program services, connecting individuals with needed resources, including Medicaid services, medical, social, educational, and other resources.
  • Adhere to the CareStar Rule and understand and comply with CareStar Policies and Procedures.
  • Maintain confidentiality and adhere to the Acceptable Use Policy while using CareStar's information systems.
Requirements:
  • Bachelor's Degree in social work, counseling, psychology, or a related field; or Licensed Social Worker (LSW).
  • Minimum of one year post-degree experience working with individuals with diagnosed chronic conditions, or minimum of three years' post-degree experience working with individuals with severe and persistent mental illness.
  • Knowledge of issues affecting people with severe and persistent mental illness and community-based interventions/resources for those individuals.
