Clinical Pharmacy Consultant

Position Overview:
Woodmark Pharmacy has been a leader in providing comprehensive pharmacy services to long-term care facilities and senior living communities for over two decades. Our dedicated team of certified geriatric pharmacists collaborates with clinical and financial leaders to tailor programs that meet the unique needs of our clients. We prioritize the integration of existing systems and the adoption of innovative technologies to ensure the safety and efficiency of care for residents and patients.
Clinical Pharmacist Consultant Role:
The Clinical Pharmacist Consultant plays a crucial role in assessing and reporting on patient medication regimens, ensuring they are appropriate, effective, and safe. This includes considering quality of life, potential adverse effects, cost implications, and compliance with regulations.
Key Responsibilities:
- Assist the Director of Clinical Pharmacy Services & Education in developing new resources for clients and covering for the consulting team during absences.
- Conduct reviews for approximately 50-60 beds daily, totaling around beds monthly for medication regimen evaluations.
- Perform audits at nursing stations, including medication room assessments, medication cart inspections, and controlled substance documentation checks.
- Review changes in patient conditions and new admissions.
- Engage in facility Quality Assurance and Performance Improvement (QAPI) meetings.
- Provide support during Department of Public Health (DPH) surveys.
- Lead clinical initiatives such as antimicrobial stewardship, opioid stewardship, psychotropic stewardship, and anticoagulant stewardship.
- Exhibit strong customer service skills and effectively communicate with visitors, staff, and colleagues.
- Demonstrate excellent organizational abilities, managing multiple responsibilities and reporting as needed.
- Maintain a high level of accuracy and attention to detail.
- Proficient in reading, writing, and communicating in English.
- Possess strong computer skills, particularly in Microsoft Office.
Qualifications:
- BS RPh or PharmD degree.
- A minimum of 1-3 years of experience as a Long-Term Care Consultant is required.
- Exceptional communication and customer service skills.
- MA Licensing is mandatory.
- This role necessitates regular interaction with residents, coworkers, and supervisors, requiring proficiency in English at an intermediate or advanced level.
