About the Role

We are seeking a skilled Transition Coordinator II to join our Care Management Team at Alliance. As a key member of our team, you will play a critical role in providing Transitional Care Management and Physical Health Consultation for members with complex needs.

Key Responsibilities
  • Provide Care Team Support: Collaborate with care teams to develop and implement comprehensive care plans that address the unique needs of each member.
  • Transition Coordination: Conduct on-site visits, outreach to providers, and facilitate clinical handoffs to ensure seamless transitions for members.
  • Case Management: Develop and implement 90-day post-discharge transition plans, ensuring members have access to necessary services and supports.
  • Monitoring and Coordination: Escalate high-risk cases, review complex cases with supervisors, and obtain information releases to improve care management activities.
  • Documentation: Ensure accurate and timely documentation of clinical information, meeting state and agency requirements.
Requirements
  • Education: Graduation from an accredited school of Nursing or Master's degree in Human Services or related field.
  • Experience: At least 3 years of experience in care management, case management, or discharge planning in a behavioral health or medical setting.
  • Licensure: Valid, active RN license in North Carolina or valid, active clinical license (LCSW, LMFT, LCAS, LCMHC, LPA) in North Carolina.
