Key Responsibilities- Comprehensively plan for case management services to ensure optimal patient outcomes
- Carry out activities related to utilization management, discharge planning, care coordination, and referral to other levels of care
- Collaborate with physicians, social workers, and interdisciplinary teams to facilitate clinical pathways and achieve desired treatment outcomes
- Promote interdisciplinary collaboration and champion service excellence
- Work collaboratively to ensure patient needs are met and care delivery is coordinated across the continuum at the appropriate level of care
- 3 years of direct patient care experience in an acute care hospital in a leadership role
- Bachelor's Degree in Nursing or healthcare-related field
- ~1 year of case management or utilization management experience
