Job Overview

This position requires a thorough understanding of our treatment program and philosophy, as well as excellent communication and leadership skills. As a Nurse Care Coordinator, you will be responsible for coordinating patient care, leading a multidisciplinary team, and ensuring that patients receive high-quality, individualized care.

Key Responsibilities
  • Coordinate patient care services, including assessing patient needs, developing care plans, and implementing interventions.
  • Lead a multidisciplinary team, collaborating with physicians, therapists, and other healthcare professionals to ensure comprehensive patient care.
  • Develop and implement policies and procedures to promote safe and effective care practices.
  • Participate in quality improvement initiatives, analyzing data and identifying opportunities for improvement.
  • Maintain accurate and detailed records of patient care, adhering to confidentiality and HIPAA guidelines.
Requirements
  • Current RN license in the state of operation
  • Bachelor's degree in Nursing or related field
  • Minimum 2 years of experience in a healthcare setting, preferably in a leadership or coordination role
  • Knowledge of addiction and mental health treatment principles and practices
  • Excellent communication, leadership, and problem-solving skills
