Job Summary

We are seeking a Clinical Care Specialist to join our team at Wheeler Clinic in New Britain, CT. As a key member of our integrated healthcare team, you will provide clinical and case management services to children and adults across the lifespan.

About the Role

The successful candidate will work as part of a multidisciplinary team to deliver high-quality care that meets the behavioral, primary, and dental health needs of clients. This includes providing behavioral health consultation to primary care providers, assisting clients with medical treatment compliance, and referring clients to other Wheeler services or community resources as needed.

Key Responsibilities
  • Conduct program-specific screening and evaluation, formulating diagnoses in accordance with the Diagnostic Statistical Manual (DSM).
  • Develop and document clear clinical formulations and treatment plans that are individualized and measurable, developed in collaboration with clients/families and others as relevant.
  • Demonstrate knowledge of psychopathology and other behavioral health concerns, as well as primary modalities of psychotherapeutic intervention.
  • Triage clinical emergencies and implement program-specific safety and consultation protocols.
  • Communicate effectively with clients, families, and internal/external providers.
  • Meet direct service productivity goals as defined by the program.
Requirements

To be considered for this role, you must possess a Master's degree in Marriage and Family Therapy, Counseling, Social Work, or a related license-eligible field. You should also be bilingual in Spanish and English and hold one of the following credentials: LMSW, LCSW, LMFTA, LMFT, LPCA, LPC, or licensed/license-eligible Psychologist in the State of Connecticut.
