Job Summary

The Integrated Health Care Specialist will provide clinical and case management services for children and adults across the lifespan in an integrated health care setting. This position requires a master's degree in a related field, bilingual skills in Spanish and English, and licensure as a therapist or counselor.

Key Responsibilities:

  • Provide behavioral health consultation to primary care providers and assist clients in meeting medical treatment compliance.
  • Develop and document individualized and measurable treatment plans in collaboration with clients and other stakeholders.
  • Triage clinical emergencies and implement program-specific safety and consultation protocols.

Requirements:

  • Master's degree in Marriage and Family Therapy, Counseling, Social Work, or related license-eligible field.
  • Bilingual in Spanish and English.
  • Licensure as a therapist or counselor in the State of Connecticut.

Salary Range: $65,000 - $85,000 per year, depending on experience.

This is a fee-for-service position based in New Britain, CT.
