**Job Summary:**

Under general supervision, the Clinical Social Worker II provides therapeutic intervention and social work services to patients and their families to enhance comprehensive, integrated, and uninterrupted care in the hospital and to have continuity of care in the community.

**Key Responsibilities:**

* Provides therapeutic intervention and social work services to patients and their families
* Demonstrates positive interpersonal relations with patients, families, visitors, and co-workers
* Mobilizes resources to reduce risk and serves as an ambassador between hospital and community
* Collaborates with healthcare teams to develop and implement comprehensive care plans
* Provides education and support to patients and families on health-related issues

**Requirements:**

* Master's degree in Social Work (MSW) required
* Maryland state LCSW-C licensure and credentials commensurate with clinical assignment
* Two years of social work experience in an acute hospital setting preferred
* Pediatric experience required
