Clinical Social Worker Job Summary

The Clinical Social Worker provides comprehensive social work services to patients and their families, with a focus on patient safety, customer service, and family-centered care. This role requires a strong understanding of normal growth and development, family functioning, and the impact of chronic illness/disability on patients and families.

Key Responsibilities:
  • Conduct psychosocial assessments of families and participate in the development of interdisciplinary treatment plans.
  • Provide educational, supportive, and counseling services to families, as well as coordinate services with community agencies.
  • Participate in discharge planning and patient post-discharge follow-up, with a focus on achieving high-quality and cost-effective patient outcomes.
  • Work effectively with an economically and culturally diverse population, balancing patient/family needs with constraints imposed by third-party payers and agency regulations.
Requirements:
  • Masters Degree in Social Work from a University accredited by the Council on Social Work Education.
  • Current license to practice Social Work in the State of Maryland, with LCSW-C required.
  • Five years of experience as a Social Worker in an acute or rehabilitation hospital, with pediatric experience preferred.
Skills and Abilities:
  • Knowledge of normal growth and development for infants to young adults.
  • Ability to effectively communicate with interdisciplinary team members, family members, community agencies, referral sources, and payers.
  • Ability to work effectively with an economically and culturally diverse population.
Working Conditions:

Frequent stress due to multiple demands, shifting priorities, shifting populations, and tight time frames. Risk of exposure to body fluids, infectious waste, or blood-borne disease.
