Job Summary:

This position is a key member of the healthcare team, responsible for coordinating care for members of a defined population. The ideal candidate will have a strong background in social work and case management, with experience working with diverse populations. The Clinical Social Worker - Case Manager will work closely with the healthcare team to develop and implement individualized plans of care, ensuring that members receive the most effective and efficient care possible. The role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team. The Clinical Social Worker - Case Manager will also be responsible for identifying and addressing social determinants of health, such as housing and food insecurity, and connecting members with community resources to support their overall well-being.

Essential Responsibilities:

  • Develop and implement individualized plans of care for members, taking into account their unique needs and circumstances.
  • Collaborate with the healthcare team to identify and address social determinants of health, such as housing and food insecurity.
  • Connect members with community resources to support their overall well-being.
  • Provide education and support to members and their families to help them navigate the healthcare system.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed.
  • Document all interactions with members and the healthcare team in a timely and accurate manner.


Basic Qualifications:

  • Master's degree in social work (MSW) required.
  • Minimum three (3) years of clinical experience plus two (2) years in case management required.
  • Minimum one (1) year of experience working with the defined population required.


Preferred Qualifications:

  • Experience with computer software programs in a Windows environment preferred.
  • Knowledge of community systems and resources in the defined service area preferred.
  • Knowledge of regulatory issues for the Mid-Atlantic area preferred.


Work Environment:

This position is based at Largo Medical Center, with occasional travel to other Kaiser Permanente facilities. The ideal candidate will be able to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
