Clinical Social Worker Job Summary

The Clinical Social Worker provides comprehensive social work services for the patient population served and care management services to those patients in clinical programs.

Key Responsibilities:
  • Conduct psychosocial assessments of families to identify their needs and develop a plan to address them.
  • Participate in the development of an interdisciplinary treatment plan to ensure that the patient's needs are met.
  • Provide educational, supportive, and counseling services to families to help them cope with their situation.
  • Coordinate services with community agencies to ensure that the patient receives the necessary support.
  • Participate in discharge planning and patient post-discharge follow-up to ensure a smooth transition.
  • Achieve high-quality and cost-effective patient outcomes across the continuum of care.
  • Function with a focus on patient safety, customer service, and family-centered care.
  • Participate in performance improvement initiatives for the department.
Requirements:
  • Masters Degree in Social Work from a University accredited by the Council on Social Work Education.
  • Current license to practice Social Work in the State of Maryland. LCSW-C required.
  • Five years experience as a Social Worker in an acute or rehabilitation hospital with pediatric experience preferred.
  • Experience in child welfare, family service, or mental health setting, is also desirable.
Knowledge, Skills, and Abilities:
  • Knowledge of normal growth and development for infants to young adults.
  • Knowledge of family functioning and the nature of chronic illness/disability and its impact on the patient and family.
  • Skill in individual, family, and group treatment modalities.
  • Ability to effectively communicate both orally and in writing with interdisciplinary team members, family members, community agencies, referral sources, and payers.
  • Ability to work effectively with an economically and culturally diverse population.
  • Ability to balance patient/family needs with constraints imposed by third-party payers and agency regulations.
  • Knowledge of third-party payer systems including Medicaid and Managed Care Organizations.
