Job Summary

This role involves working as part of a multidisciplinary team to deliver comprehensive care to patients in an outpatient setting. The ideal candidate will have strong communication skills, empathy, and a passion for delivering individualized care.

Key Responsibilities:

  • Providing direct patient care under the supervision of a Registered Nurse
  • Conducting assessments, implementing treatment plans, and monitoring patient progress
  • Maintaining accurate records and reporting any concerns to the healthcare team
  • Collaborating with other healthcare professionals to ensure seamless patient care

Requirements:

  • Curren LPN-LVN license in the state of practice required
  • CPR certification required
  • IV certification required if required by state regulations
  • High school diploma or equivalent required
  • Completed training program approved by the medical director and the governing body as required by CMS guidelines prior to operating the water treatment system

Compensation and Benefits

The salary range for this position is $62,000-$85,000 per year, depending on experience and qualifications.
